SECTION A - All items must be completed under Section A to be eligible for any award level and to be considered a "unit in good standing" with the PTA Council, MD PTA, and National PTA. ____ 1. At least one delegate attended all Council meetings (delegate must sign attendance book) OR contacted the Council Secretary to obtain information for any meetings missed. The delegate reported to the unit executive board and/or general membership regarding Council activities and information. (Attach copy of one report). ____ 2. The Purposes of the PTA were read or displayed at all General and Board of Directors meetings. (Attach a copy of the minutes from one meeting) ____ 3. Dues payments are expected monthly! The first dues payment must
be received (not postmarked) by the PTA Council and MD PTA for all current
members by October 31, 2007. Mail check, made payable to the PTA Council
of Frederick County, (for $.75 per member) to: Daphne Gabb, Treasurer
P.O. Box 1368 Middletown, MD 21769 ____ 4. Local unit is incorporated. You may go to this web site to check your unit's status: http://sdatcert3.resiusa.org/ucc-charter/CharterSearch_f.asp. ____ 5. Names, addresses, phone numbers, and e-mail addresses of all officers and Council delegates for the upcoming year must be received by PTA Council and the MD PTA by July 1, 2007 so that the unit can receive all correspondence. E-mail MD PTA at office@mdpta.org or via regular mail to 5 Central Ave., Glen Burnie, MD 21061. ____ 6. Submit a copy of the annual treasurer's audit to the MD PTA office by September 30, 2007 (attach a copy of the audit). ____ 7. Have an Employer Identification Number from the IRS, EIN#_____________________. ____ 8. File appropriate tax forms with the IRS and the MD Revenue Administration
Division or the MD State Department of Assessments and Taxation. (attach
copy of 1st pg. or signature pg.) ____ 10. Have a current approved copy of the unit's bylaws and provide a copy to all Board of Directors members. Date bylaws approved: ________________________________ ____ 11. President and Treasurer received Council or MD PTA training. ____ 12. Insurance premium paid prior to 5/5/08. Unit must have current updated and paid liability and bonding insurance in order to be a unit in good standing. Check # _______ and date written_______________________. ________ TOTAL FROM SECTION A SECTION B- At least six (6) items from section B and all 12 items from section A must be completed to earn a Certificate of Distinction. ____ 13. Final State/National dues were received (not postmarked) in
the MD PTA office by March 30, 2008. All unused or damaged membership
cards were returned to the MD PTA office. Mail a check made payable to
the MD PTA for both the state and national dues ($3.25 per member). Mail
this check to: Maryland PTA ____ 14. The final Council dues payment must be received (not postmarked)
by the PTA Council for all members by March 30, 2008. Mail check made
payable to the PTA Council of Frederick County (for $.75 per member) to:

Conference or Annual Fall Convention. ____ 17. Held at least three general membership meetings (or number required by local bylaws) during the year. (Attach agenda from one meeting) ____ 18. PTA budget was presented to and adopted by general membership
at a general membership meeting. (Attach copy of the budget) ____ 19. Held at least three Board of Directors meetings during the year (members are listed in the unit's bylaws). (Attach agenda from one) ____ 20. Local unit procedure books and reference materials (Cash Encounters, Quick Reference Guides, Money Matters, etc.) were made available to all Board of Directors. ____ 21. Plans of Work were submitted by committee chairmen and approved by Board of Directors. (Attach copies of two plans) ________ TOTAL FROM SECTION B SECTION C- At least four (4) items from section C, at least six (6) items from Section B, and all 12 items under section A must be completed to earn an Award of Excellence. ____ 22. Unit has active Family Involvement Committee. (Attach copy of
Plan of Work) ____ 23. Communicated regularly with general membership via newsletter, electronic messaging, phone tree, etc. (Attach a printed copy of one communication or a copy of the plan of work for the Communications Committee) ____ 24. At least three members of the Executive Board subscribed to the Maryland PTA Bulletin. (Attach copy of enrollment form) ____ 25. At least three members of the Executive Board subscribed to "Our Children" magazine. (Attach copy of enrollment form) ____ 26. Unit Treasurer attends MD PTA State Treasurer's training (this
is an all day course). ____ 27. Unit had an Advocacy Committee or advocated actively on an issue approved by the Board. (Attach a copy of Plan of Work or the minutes) ____ 28. At least one delegate attended a MD PTA or Council legislative
conference, meeting, or workshop (such as PTA Night in Annapolis, Council
