SECTION A - All items must be completed under Section A to be eligible for any award level and to be considered a "unit in good standing" with the PTA Council, MD PTA, and National PTA.

Names, addresses, phone numbers, and e-mail addresses of all officers and Council delegates for the upcoming year must be received by PTA Council and the MD PTA by July 1, 2007 so that the unit can receive all correspondence. ____ 6. Submit a copy of the annual treasurer's audit to the MD PTA office by September 30, 2007 (attach a copy of the audit).

____ 8. File appropriate tax forms with the IRS and the MD Revenue Administration Division or the MD State Department of Assessments and Taxation. (attach copy of 1st pg. or signature pg.)
